Report a Concern

The Online Reporting Portal allows reporters to report suspected instances of child abuse or neglect.

Providing complete and accurate information about the situation and circumstances may help avoid delays in assessing the safety and well-being of the child or children. Incomplete information may delay review or result in the report being screened out.

Request Services

The Service Request Portal allows interested individuals and organizations to submit a request for services on behalf of a child.

Eligibility is based on federally established criteria and will be verified by Health and Human Services. Applications may be submitted by a parent or caregiver, public or private agency, treatment provider, tribal social services, juvenile court, or another authorized party.
